HOW TO CLAIM YOUR REBATE:

1. Purchase a qualifying EOTech Holographic Weapon Sight or Magnifier between 6/4/2014 and 8/15/2014. (See list of qualifying products at http://EOTECH.rebateaccess.com.)
2. Go to http://EOTECH.rebateaccess.com to register your rebate (details below).

In order to obtain a rebate, visit http://EOTECH.rebateaccess.com and enter the information requested to complete your rebate registration form online.
You must have internet access, a valid email address, and a printer in order to register your rebate.

ONLINE SUBMISSION DETAILS:

1. Complete and print the online rebate registration form at https://eotech.rebateaccess.com/.
2. Enclose a photocopy of the entire UPC barcode and Serial Number label found on the package sleeve or end of the black case.
3. Enclose PROOF OF PURCHASE REQUIREMENTS:

  • Completed Online Registration Form
  • Photocopy of entire UPC Barcode & Serial Number label from the product packaging
  • Cut out original Inspection Sticker located on the packaging sleeve
  • Original Sales Receipt*

4. Mail all of these items to the address indicated on the form you print from the website.
5. Submission must be postmarked by 8/31/2014 in order to qualify.

MAIL-IN PROCEDURE:

In order to expedite your rebate, it is recommended that you use the online rebate registration. If you do not have online access, you may print the attached PDF and submit the information requested below. Enclose this rebate form, photocopy of the UPC barcode & serial number,original sales receipt* and cut out of the original inspection sticker located on the sleeve. Make sure to include your name, street address, apt number, city, state, zip, phone, email and mail to the following address: EOTech SIGHT-IN SAVINGS REBATE, Promotion #56182, PO Box 22092, Tempe, AZ, 85285-2092.

IMPORTANT: Keep copies of all materials submitted for your records. Materials submitted cannot be returned.

TERMS AND CONDITIONS:

EOTech’s Rebate is valid only for retail sales of new eligible products purchased through EOTech authorized dealers and distributors (No EBay or Amazon Purchases) and sold to consumer purchaser’s by EOTech authorized dealers from 6/4/2014 through 8/15/14. All submissions must be postmarked by August 31, 2014. Submissions posted after that date will be rejected. Requests from PO Boxes are not allowed (except in ND). * Sales receipt must include date of purchase, eligible product(s) purchased, as well as the name of the EOTech authorized dealer who sold the eligible products(s). Unidentifiable user names from auction sites such as EBay will not be accepted. Dealer/distributor/department purchases will not be accepted. HHS I and HHS II models qualify as one rebate. WARNING – Products for which rebate is claimed may not be returned. Failure to submit any of the requested documentation will delay or prevent rebate issuance. Fraudulent submissions of multiple requests could result in federal prosecution under the U.S. Mail Fraud Statutes (18 USC, Section 1341 and 1342).
